CASE :  A 23 year old man, resident of Nakrekal and goldsmith by occupation, has come to the OPD with chief complaints of fever and backache since 6 days.  History of Present Illness:   The patient was apparently asymptomatic 6 days ago, then he developed fever (high grade) which was continuous, not associated with chills and rigor.
