23/M with fever and backache

 This is an online E log book to discuss our patient's de-identified health data shared after taking his/her/guardian's signed informed consent.Here we discuss our individual patient's problems through series of inputs from available global online community of experts with an aim to solve those patient's clinical problems with collective current best evidence based inputs. CASE :  A 23 year old man, resident of Nakrekal and goldsmith by occupation, has come to the OPD with chief complaints of fever and backache since 6 days.  History of Present Illness:   The patient was apparently asymptomatic 6 days ago, then he developed fever (high grade) which was continuous, not associated with chills and rigor. It got subsided on taking medication given by a local RMP. He also complains of backache since 6 days which was insidious in onset, gradually
